Responsibilities:

• Engage community social networks to collaborate with YLDHC activities
• Coordinate, organize, and participate in scheduled outreach activities to facilitate member enrolment for DHC programs
• Conduct patient empowerment programs for individuals or in groups
• Assist in the promotion and publicity activities of the DHC
• May need to take on flexible hours and occasional Evening, Saturday or Sunday duties

Requirements:

• Registration with the Social Workers Registration Board
• 2 or more aggregate years of practice as Assistant Social Work Officer
• Knowledgeable in community resources and potential collaborators
• Experience in handling cases with chronic diseases and conducting patient empowerment programs will be an advantage
