高級護理統籌主任(護士)(編號: YLDHC/SCC)

Responsibilities:

• Perform health assessment and screening for registered clients of the Yuen Long District Health Centre (YLDHC);
• Conduct Nurse Clinic for client clinical care, including conducting complication screening, coaching, and chronic disease management;
• Act as a case manager to organize and coordinate care services from Family Doctors and Network Health Services professionals as appropriate;
• Report to the Chief Care Coordinator of YLDHC, assist in developing education resources and training, and lead the nurse team
• Participate in promotional and outreach activities, including supervision of subordinates in planning and activities
• Night duty is not required, but may need to take on flexible hours and Evening, Saturday, or Sunday duties as needed.
• May be required to rotate among various centres in the Yuen Long District.

Requirements:

• Degree in Nursing and registered with the Nursing Council of Hong Kong;
• More than five aggregate years of experience in patient care related nursing practice and 2 years in management role
• Working experience in Medicine & geriatric/ Women Health/ Primary Health Care Nursing / Community Nursing will be an advantage
• Fluent in Chinese, English, and preferably, communicative in Mandarin
• Keen in teamwork, proactive, skilled in persuasive techniques, and possess good computer literacy
• Location: Yuen Long
