護理統籌主任(護士)(編號：YLDHC/CC)

Responsibilities:

• Perform health assessments for registered clients of the Yuen Long District Health Centre (YLDHC)
• Organize clinical care for clients with health risk factors from network medical practitioners
• Conduct complication screening, coaching and disease management to patients diagnosed with Prediabetes/Diabetes mellitus and/or Hypertension
• Act as a case manager to organize and coordinate care services from network health services professionals as appropriate
• Report to the Chief Care Coordinator of YLDHC and assist in promotional and outreach activities as assigned
• Night duty is not required; but may need to take on flexible hours and Saturday or Sunday duties as needed
• May be required to rotate among various centres in Yuen Long

Requirements:

• Degree in Nursing and registered with the Nursing Council of Hong Kong
• 5 aggregate years of experience in patient care related nursing practice as a Registered Nurse
• More experienced nurses may be offered a more senior rank
